Relocation Services Coordination
Relocation Services Coordination (RSC) is a medical assistance (MA) reimbursed case management service established to provide coordination of activities to help a person who resides in eligible institutions to gain access to medical, social, educational, financial, housing and other needed supports and services to move from institution to the community based setting.
Moving Home Minnesota
Moving Home Minnesota helps people on Medical Assistance (MA) move out of institutions. It helps pay for services that your MA or MA-Waiver benefits will not pay for, provides counseling, and makes sure that your transition to living in the community goes smoothly.
Housing Access Coordination
Housing access coordination (HAC) is a waiver service that helps a person plan for, find and move to his/her own home. It is a pay-for-performance, person-centered service that pays for staff assistance based on the results achieved. A provider is reimbursed for actual time spent helping a person get housing.
Transitional services covers items and expenses necessary and reasonable for a person to transition from an eligible setting to his/her own home
